RESUMO

Tattooing is becoming more popular around the world. As a result, the reported complications of tattoo are increasing. Local inflammation, infection and allergic reactions are the most common adverse effects. We report here on a healthy 39-year-old woman who developed multiple verruca plana lesions after undergoing semipermanent tattooing. The tattooing had been done 1 year earlier by an amateur.

BACKGROUND: Mycosis fungoides (MF) is the most frequent form of cutaneous lymphoma. Early stage MF is known to be responsible to the various topical and systemic therapies. However, there have been few clinical reviews of the treatment modalities of the early stoge MF in Korea. OBJECTIVE: The aim of this study is to investigate the therapeutic response of early MF to different treatment modalities. METHODS: The medical records, clinical photographs, and biopsied tissues, of fifty three cases of early MF were reviewed. RESULTS: The mean age at diagnosis of early MF was 43.0 years old (varied between 9 and 81 years old). Twenty three (43.4%) were male and thirty (56.6%) were female. Topical carmustine (bis-chloroethyl-nitrosourea, BCNU) was the most commonly used treatment modality in the early MF (34.0%). Patients treated by BCNU in stage IA ( or =10% skin involved). Even Psoralen plus UVA (PUVA) therapy, which was most efficient in stage IB, revealed a high relapse rate after one year. CONCLUSION: BCNU has been most commonly used for the treatment of early MF during the past 10 years in the authors' hospital. It showed a comparatively high response rate, and the relapse rate of BCNU was lower than for other therapies in stage IA. However many treatments became ineffective when MF progressed to stage IB. Therefore the authors confirm that early diagnosis and proper therapy are most important for the treatment of early MF.

RESUMO

rcinoma (SCC) is the second most common skin cancer. In particular, invasive SCC has a high risk of metastasis and sometimes, this can be fatal. At present, the treatment of choice for invasive SCC is complete excision with a proper surgical margin, but most cases of SCC develop on cosmetically-sensitive areas, so sometimes significant scarring or deformities can lead to an unhappy result. An 80 year-old female presented with a 4x1.5cm sized SCC on her lower lip. Although surgery was the first line therapy for this invasive SCC, a cosmetically poor outcome was expected. Thus the patient did not want to undergo a surgical operation, so we had to explore other treatment options. Recently there have been many reports that show imiquimod 5% cream is effective in the treatment of invasive SCC. Therefore we started topical therapy with imiquimod 5% cream five times a week. After 6 months, the lesion size was reduced to 2x1.5cm and we could remove the remnant tumor by wedge resection with cosmetically acceptable result. We suggest this neo-adjuvant immunotherapy can be an alternative for the treatment of cosmetically-critical SCC and present this case as a good example that has shown successful size-reductive neoadjuvant immunotherapy using imiquimod in invasive SCC.

BACKGROUND: Hair loss is a very common disorder and the number of patients is known to be increasing. Female pattern hair loss (FPHL) is not only a medical problem but also a very severe psychosocial problem for many female patients. Anti-androgen drugs, mineral supplements and topical minoxidil have all been used for the treatment of FPHL. However they do not always achieve successful results and there is still much need for more effective therapy. OBJECTIVE: The purpose of this 18-week, double-blind, placebo-controlled, randomized clinical trial was to investigate the efficacy and safety of a new topical agent, AP-FHG0604T, in the treatment of FPHL. METHODS: A total of 33 women with FPHL (mean age: 33.4 years old) applied either a topical AP-FHG0604T solution (n=17), or placebo (vehicle for AP-FHG0604T solution; n=16) twice daily. Efficacy was evaluated by phototrichogram, investigator's photographic and patient's subjective assessments. All adverse effects were reported during the study. RESULTS: After 18 weeks of therapy, topical AP-FHG0604T treatment showed a significant improvement compared to baseline values of total hair count, non-vellus hair count, and linear hair growth rate. In the placebo group, non-vellus hair count and ratio of anagen hair significantly decreased. The change rates of total hair count and non-vellus hair count in the AP-FHG0604T group were significantly higher than those in the placebo group. Neither investigator's photographic assessments nor patient's subjective assessments of hair growth showed statistically significant differences between the AP-FHG0604T group or the placebo group. Some patients who used AP- FHG0604T complained of local irritation during the study, but the irritation was so mild that they did not need any treatment for this. CONCLUSION: We conclude that AP-FHG0604T is a safe and efficient topical agent which can be used as another treatment of choice for FPHL, as shown by objective assessment with phototrichogram.

RESUMO

Pyomyositis is an acute, suppurative bacterial infection of striated muscle. It occurs primarily, and not from contiguous infection. It is considered rare in temperate climates. There has been no report of pyomyositis in the Korean dermatologic literature to date. We report a case of pyomyositis which developed in a 53-year-old male. The patient was admitted for the treatment of cutaneous leukocytoclastic vasculitis on his entire body. He was treated with oral corticosteroids for approximately three weeks. The skin condition did show improvement; however, the patient developed a persistent fever and a newly-found mass on the right upper back. The mass was diagnosed as an abscess, replacing the striated muscles of the back by sono-guided needle aspiration. After surgical intervention and IV antibiotic treatment, the symptoms and mass on the back disappeared. We speculate that the underlying skin condition and long-term use of oral corticosteroids predisposed the patient to pyomyositis.

RESUMO

Endometriosis is the aberrant presence of endometrial tissue outside the uterine cavity. We report a case of cutaneous endometriosis which developed on the scar of a uterine myomectomy in a 35-year-old female. The patient presented with a bean-sized, oval-shaped, erythematous nodule, which was located where a drain tube had been inserted. During her first visit to our clinic, she felt slight pain upon light touch to the lesion. She was not menstruating at this time. The lesion had been getting bigger and she reported that the pain was aggravated during menstruation. Diagnosis was confirmed by histopathologic examination, which showed multiple endometrial glands with decidualized stroma, and hemosiderin pigment. Many dermatologists are not familiar with cutaneous endometriosis. Therefore, we present this case as a good example which shows pathognomonic symptoms and characteristic histopathologic findings of cutaneous endometriosis.

Incontinentia pigmenti (IP) is an uncommon genodermatosis that usually occurs in female infants. It is characterized by ectodermal, mesodermal, neurological, ocular, and dental manifestations. The aim of this study was to clarify clinical symptoms, accompanying diseases, and complications of IP. Forty cases of IP have been reviewed by their medical records, laboratory data, clinical photographs, and telephone survey. Male-to-female ratio was 1 to 19 and their onsets were mostly in utero. They were usually diagnosed during the neonatal period owing to their early expression of skin manifestation. Central nervous system anomalies were found in 46.7%. Ocular disorders and dental defects were detected in 66.7% and 72.7% respectively. The most commonly diagnosed anomalies were hypodontia, retinopathy, and seizure. For better understanding of IP, long term and close cooperation between dermatologists, pediatricians, neuroscientists, genentic counselors, and even dentists is crucial.

RESUMO

Livedo vasculitis is a chronic dermatosis characterized by recurrent painful ulceration of the lower limbs, which heals to leave atrophie blanche. The precise pathophysiology is not known. Antiplatelet, anticoagulant, fibrinolytic therapies and anabolic steroids have been reported to be helpful in this syndrome. However, no consistent benefit has been demonstrated with any treatment modality. Recently, pulsed intravenous immunoglobulin therapy has been reported to be effective in some refractory cases. We herein report two cases of recalcitrant livedo vasculitis which were effectively treated with pulsed intravenous immunoglobulin therapy. These were the first trials carried out in Korea.

Incontinentia pigmenti (IP) is an uncommon genodermatosis that occurs mostly in female infants, but is rarely found in male infants. Male patients with incontinentia pigmenti are usually more severely affected than their female counterparts. IP is characterized by ectodermal, mesodermal, neurological, ocular, and dental manifestations. Herein, we report a case of IP in a male infant who presented with a typical course of skin manifestation, dental defects, and recurrent partial seizures. However, he did not show any signs or symptoms for ocular or cardiovascular anomalies.
